The Cross-NIHR-Collaboration for Multiple Long-Term Conditions

This work is funded by a Team Science award from the National Institute for Health and Care Research for the cross-NIHR collaboration for Multiple Long-Term Conditions (MLTC) – Models of Care workstream.

Models of Care is one of the three workstreams of the collaboration which brings together NIHR translational, applied and policy infrastructure; to build capability, capacity and leadership and address challenges in the field of MLTC research.

The MLTC Models of Care workstream

We are a diverse, multidisciplinary group. We have complementary professional backgrounds, lived experience, methodological and research expertise spanning areas of importance in MLTC research. The further development of our interdisciplinary team is being nurtured through regular workstream meetings, team seminars involving external speakers, and collaborative online spaces – including the meetings for the core project:

Projects

There are two core projects attached to the workstream, with the Team Science project being led from Exeter:

Our work is a national and international priority. We are addressing the NIHR call for Models of Care for MLTC by seeking a way of delivering health and care services that ensures the best possible outcomes for individuals and groups of people living with MLTC.

We will apply the NIHR strategic framework for MLTC and ‘define the principles, values, roles, and practices that guide how care and support are provided throughout the stages of MLTC’.
