We can’t wait to launch this new platform to improve accessibility of cardiac rehab – we look forward to hearing from you soon!

The Digital version of REACH-HF provides an additional mode of delivery to maximise the choice for patients, thereby improving access and uptake of cardiac rehab by your patients. We now have 4 sites in our Early Adopter programme and we are currently setting them up to go live over the next couple of months; welcome to Guy’s and St Thomas’, Southern Healthcare, Southampton and Provide Essex. We look forward to working with you in the months to come.
